Your ISP may be using a NAT or proxy server of some sort. I have had players with the same IP and ISP but did not know each other.
Check that your IP is what you think it is. Here is 1 method to determine what the rest of us would think your IP address is. http://whatismyipaddress.com/ If the wrong IP address shows up, you can't host a server unless your ISP helps you. Unlikly IMO. If the wrong IP address shows up, have your friends also try. If they have the same ISP, it may show the same IP address for them as it does for you.
