I see other posts relating to this topic but I cannot find a definitive
answer. We wish to use Imail as a gateway for "incoming" mail only in
front of Exchange. We have created only a bogus host name within Imail
called bogus.com.
?? why? Setting up Imail relaying has nothing to do with Imail domains. bogus.com is bogus step.

Modified the MX record for the real domain
... to be relayed.

to point to
the Imail box. Created a host file entry on the Imail box to point to
the Exchange box (MX resolution on the Imail box for the real domain
correctly points to the Exchange box as a result).
You are now finished with setting up IMail for relaying inbound traffic to the Exchange box.

 We have enabled
"Relay Mail for" and included the IP of the Exchange box.
this step is unnecessary since you said Exchange is delivering its own mail to Internet

Yet email destined for the real domain is still denied "not a gateway" at the Imail box.
hosts must have

ip.ad.re.ss realdomain.com

each field has its own purpose

1. RHS tells Imail which @recipient.domain to accept mail for

2. LHS say to which ip is the "next-hop" to relay the mail for @recipient.domain (no DNS lookups)

 According to the manual:

Setting Up IMail Server as a Mail Gateway
If you are using any of the "Relay Mail for" options on the SMTP
Security tab, and want to relay outgoing mail for another mail server,
... you said you are not doing this, so this setup section does not apply.

the only thing the "relay mail for" option restricts are the IP's of the
SMTP client allowed to "send into" the Imail server, not "receive from"
the Imail server. How do I allow Imail to relay for a specific
destination without opening it up for the world?
you've done that in hosts file

FYI, if I do open up to "relay mail for anyone" mail flows correctly.
ok. Imail does not seem to be reading the hosts file or you have an error in the hosts file. rename hosts file and then re-create the hosts file using notepad.exe, not Word.

bogus.com was a bogus side-show. you can remove it.
