#40: LilyPond already (before this patch) places \voiceOne r2 on the top
line (and the book by Tedd Ross says to put them on the top line for what
that is worth). Looking at engraved scores, I find about equal numbers on
top line, and one line from the top. Let's leave it as it is.
#37: on eighth rests, remember tablature has larger spacing of the
staff-lines, so the bulb on the eight rest just clears the line. I asked
about this on -user in the thread that started this issue, with no reply
yet -- unless comment #42 includes the eight rest in his positive comments.
