#41 and #44: Gould's recommendation for double-stemmed writing
is, "When both parts have rests simultaneously, ..., separate these
with at least one stave-line." But she acknowledges, "Many editions
place all minim and semibreve rests on only the outside stave-lines,
to avoid confusion." So there seems to be no definitive ruling.
