Norm: Only collision I came up with in my foggy memory was the 2001 incident between USS Newport News and the Japanese "school" vessel, Ehime Maru. I believe There was some controversy about it being caused by an "unnecessary" emergency surfacing. Nothing comes to mind with regard to WWII but that seems unlikely given numbers of ships and boats involved. Perhaps reporting not as good or covered by press blackouts at the time.
As for your other comments concerning submarine warfare in WWII, I find most ring a bell that I have seen, read or otherwise studied over the years.
