There was a famous collision between a US submarine and a cargo ship off the New England coast many years ago that set rules to this day.
It was at night and the submarine did not display the lights required by normal ships. At court the Navy claimed that they were special and didn't need to comply with the Rules. The court disagreed saying that since the submarine represented a danger to other vessels it is required to be properly lit like other vessels. Norm S/V Bandersnatch > Only collision I came up with in my foggy memory was the 2001 incident between USS Newport News and the Japanese "school" vessel, Ehime Maru. I believe There was some controversy about it being caused by an "unnecessary" emergency surfacing. Nothing comes to mind with regard to WWII but that seems unlikely given numbers of ships and boats involved.
