This seems to happen in some cases if the driver does not support
GL_EXT_packed_depth_stencil.  I was able to produce this (after fixing
another bug) when running with indirect rendering on the i915 driver.

There are a bunch of similar regressions reported in bugzilla.  Pretty
much all of the reports are against the i915 or i965 driver.
