This sounds to me like either:
1. A problem of some sort with your modem, perhaps with the initialisation
string or with the hangup string. Perhaps after going off offline your
modem is not being left in command mode. See if switching off/on your
modem between sessions helps. And look at the init strings.
2. A problem of some sort with your serial device/port.
