Ok I set the jumpers as you said (BTW the CD-ROM is a 12X Apple/Matsushita
CR-507-C SCSI CD-ROM).   There are 7 sets of pins labeled (from left to
right) :

Parity  0  1  2  (single pin) (blank) TERM PWR

As you said I jumpered Parity 0 1 and Term Pwr.

My new (used) hard drive is a 4GB IBM Ultrastar.  It's the primary (and
only) hard drive in my system.   It currently has NO OS on it.   It has pins
(from left to right):

3 2 1 0 (single pin) autostart    active term   master/slave sync   LED

write protect   autostart delay    option block mode      disable sync negot

Disable SCSI parity     disable unit attention    customize

(two horizontal pins)-- termination power enable

Right now I haven't changed any ID settings on it.  I have autostart closed,
disable sync negotiation closed, and disable unit attention closed.  I also
have the termination power enabled jumpered.

My problem is when I try to start from the CD using OS 9.1 (because I don't
have any OS on the hard drive) I get the hard drive spinning up and the
CD-ROM starting.  I get a happy face but that's it.  It just freezes after
that.  I've tried both OS 9.1 and 8.5 but without any luck.  Any ideas as to
what is happening and how I can solve this?  Thanks in advance.
