Your problem would be a lot simpler if it only related to AF. It would seem
odd that it happens in manual focus as well - when presumably it looks sharp
on the eyes through the viewfinder, but then becomes focussed elsewhere on
the final digital file. I'm not doubting that this is happening, just that I
find it difficult how it could happen (short of a manufacturing fault in the
positioning of the the CMOS) on an SLR which, by its nature, the focus point
you see through the viewfinder is what you get on the film/sensor. But
having said that, an almost idential problem appeared to occur to a number
of Canon bodies...

Sorry if this next bit sounds like I'm teaching you how to suck eggs - but
is it possible that you have your focussing on "M/A" rather than "M" - so
the (incorrect) autofocus is overiding your (correct) manual focussing ? The
only solutions I can think of for this happening on manual could be an
incorrectly set diopter (unlikely - it would make everything out of focus,
not move the focussing point) or "operator error" (again extremely unlikely
if it's happening on a regular basis or with other photographers)

A colleague of mine who uses Nikon autofocus on a daily basis for
slow-moving targets (people walking into court) claims that he has quite a
few problems with the Nikon AF on the D1-series, and now also on the D2H. In
his words it is "too clever for its own good" when it tries to predict the
path of a slow moving object, and he only uses it on single servo with none
of the predictive settings switched on.
