If you are working in 8 bit , then convert to 16 bit before doing these changes, it allows many more steps in tone and the bands will no longer be so apparent.

I have experienced banding working in 16 bits files on the "blended exposures" as described on the Luminous Landscape. They were noticeable on screen as well as prints.
I address the problem now by working on a mask created from a selection or a copy of a channel and burning them down or out using the tools darken or highlight tools with large brushes with the low hedged.
