No.  My first car with FRED / EOTD was a boxcar.  I got an HO unit from Ring 
Engineering and put the HO coupler with the device on one end of the car. 
Some S scalers still use "HO" couplers.  The battery & circuit I put inside 
the car.  Unfortunately, they are not cooperative and will not sell the unit 
without the HO truck.  It looks approximately OK.   See

The Gunderson Husky Stack I just did was based on a gift from a customer. 
He made the flasher circuit himself on some circuit board and somehow 
attached a little flasher to a little stick and the stick to a KD S- scale 
coupler.  I presume it's held on with super glue.

Both couplers appear to still be totally functional, so a switcher can 
couple on to the back of the train whenever necessary.

It's all a matter of attitude. I see lots of variety, but it helps to
accept the reality of intermodal transportation. Not all intermodal cars
are owned by TTX Co, and there are hundreds of container owners. Just new
things to learn. "No cabesse," but my latest project was to put a Flashing
Rear End Device on a double-stack car, with the 9V battery & circuit board
hidden inside the bottom container.
