Peter,

Railroad clearances vary by State and are usually legal requirements
established by each State's Public Utilities Commission.  The variences in
the real world would are not that significant in the model railroad world.
For example, side clearances only vary by about 6-inches or so.  For
railroad loading docks, the State of Missouri's legal clearance is 5'-9"
from centerline of track for any dock up to 4'-0" above top of rail.  If the
dock extends above 4'-0" then the side clearance increases to 8'-6" above
TOR.  That means loading dock doors have to be set back 8 and one-half feet
from centerline of rail.  These are minimum clearances and individual
railroads may establish their own clearances as long as they don't fall
below the legal requirement.

Keep in mind that clearances also change over the years, so steam era
clearances may be quite different than today's double stack clearances.
