I can only recommend. Users are in some cases already blocked by postscreen,
if you use it (e.g. when connecting from addreses in dynamic blacklists),
so they really must use alternative ports, and if receiving mail from server
takes a minute or two - the server doesn't care.

from, so the only address I can safely (gateway takes care of faking the
address) whitelist is therefore the header address.
is it possible to use header address in whitelisting?
Or is there any trick to whitelist such address?

on debian, you need to add user clamav in amavis group and restart clamd, so
clamd can
scan files unpacked by amavis.
maybe you need something alike.

you can also feed the mail to "spamassassin -d", it should show you if it
