The PDF has to be posted as a shared review on a cloud web storage site.
There's a size limit to what you can upload. My company is using corporate
GMAIL and I have a 30Gb limit on storage, and for now I have to attach a
shared review enabled PDF to an email to allow reviewers to join the
review*.
I haven't seen this one personally yet, or figured out how it gets
triggered. The fix for Debbie was fairly simple; she designated all of her
callout paragraph formats to straddle sideheads and columns, which
effectively masks the problem. Of course, a solution could be scripted if
necessary.
I haven't run into this recently (using FM11 and FM12 mostly) but know that it
was a bug that was NOT triggered by accidental keystrokes. The easiest fix was
to use the backup file. I can't remember if I could fix it by using the
template to reimport the master pages into the files affected.

The book is an unstructured book, but it includes 3 files that were created
from structured data. Those 3 files are still structured, because that's
the only way to keep the xref links working.

didn't, there could definitely be some extra cruft in there. You
probably checked this already, but are these files still structured?

FM11 to FM12. I
After a bit of testing, the increase seems to be linked to 3 files that
were created using DITAFMx from a batch of xml files that were generated by
script from an Excel workbook. Interestingly, these files have always been
part of the document (and inserted in the same way), so I'm not sure why

use old-fashioned 300-1200 baud modems in very isolated areas (upper Amazon,

I also found that for some unknown reason, when you save to pdf and
generate Acrobat data (PDF Setup), under bookmarks, Thread by Text Frame
always seems to be checked. Not sure what it's doing, but I found that
when it was checked the embedded videos would not play (even though I had
