When a document is converted to PDF, the PDF conversion removes all formatting and substitutes there own PDF formatting to give the appearance of the original document. All tables of contents, page numbers, paragraph numbers, indexes, captions, etc. are converted to the PDF ‘fixed’ format.

But the original poster wanted to edit that after it is open in Word and still wanted the Index, Table of Contents and page numbering to work. Does the converted Word document have the text marking for those entities? I doubt that the PDF has anything that would let Word keep the markings

You are getting exactly what you should. Legacy cannot calculate the page numbers for the TOC until the entire report/book has been generated. When sending direct to the printer that means that the TOC has to be printed last. The alternative would be to delay the start of printing until the entire
