Is it possible, without having to use layers, to specify the location
of a frame inside the margin of the header relative to the headertext?

TOC is displayed on each page. For illustration I misused the
interaction colour, which of course only works for the first page of
a chapter.

Unrelated to arranging. In fact, we have left/right/middle styles etc
but the new code has no fallbacks, so we need to add:
