the subscription should be at the DOMAIN not the FQDN or HOSTNAME. That is the SIPDOMAIN used when you ran setup? because the subscription is set to use hostname I think. The subscription should be at the SIPDOMAIN. I also assume the phone is setup to use SRV records and that those resolve correctly.
