I have similar problems with mail with no spam headers (and the
system is supposed to tag all messages, being spam or ham
with a header like this (spam = score >5, one s for each point):

X-ans.hiof.no-MailScanner-SpamScore: sss

Running Redhat 9.0, Mailscanner, SA 2.63
SA called directly from Mailscanner, no procmail,
no spamd.

but I still see couple of mail each day like this
(without that header):
