I had similar problems when I went from OS 7.6.1 to  OS 9.1 on my C500 
with the 180 MHz CPU.  I found that disk fragmentation on my HD and 
other disk errors seemed to cause the instability.  I must have run 
TechTool Pro 3 or 4 times before the C500 would accept the new system.  
Is the CD-Rom Toolkit from Intech?  Maybe you can use their free trial 
of CD/DVD Speed Tools at <http://www.IntechUSA.com/CDInfo.html>.  
Hopefully you can bring your CD-ROM back.  Then I would use a disk tool 
to see what shape the HD is in before you try the install or resurrect 
what you have now. HTH.
