Nothing Gene said spoke of consensus. And a perfect environment isn't
necessary. What Gene spoke of was buy-in. You allow every party to
have their say, and then go with the solution that makes the most
sense. If everyone has equal say and is treated fairly, you don't need
consensus to make the decision. You just make the decision.

You should read some of Patrick Lencioni's books. They're good, and
the advice works well. At least it's worked well for me and others I
know who have also read the books and applied the practices.

This sounds like a fundamental break in the 1:10:100 rule of
quality/usability. nothing can get done right if decisions are made
blindly.

Absolutely. Change is inevitable. But that's no reason not to plan
