I can see another thing happen (which I see more often the thicker the thread gets): Thread wraps are placed on top of eachother and one wrap falls down, letting the previous wraps untighten and the whole thing collapses.

I have seen this happen a lot when the Whip knot is tied behind a bead. What happens is the thread does not get tight behind the bead, it climbes up the face of the bead. What I do is push the wraps down behind the bead, with my fingernail, pull tight each time and then make the knot, then a second knot. Another thing is too many wraps and not getting each wrap tight enough. When you pull on the thread and clip it, the thread acts like a rubber band and springs back under the wraps. These are just a few common things that happen, wish I could see what you are doing so that I can help you.
