I did not find it to be a problem on a 4s running IOS 6 either and you will or may be happy to know that on the IPhone 5 VO volume on a call is louder and that is nice. I have become more convinced actually that what happens on any of them is that when vo volume is very low on a call it is because it is still being routed through the ear peace. Once it finally gets around to routing to the phone speaker on the bottom then it is better. Also, the ability to turn down the volume of the caller on a call may impact accessibility in the way you described, but it is not an accessibility issue as that is part of the main phone apart from accessibility. That is the best way I can think of to describe what I am trying to day. Some may split hairs over it, but please understand the point I am trying to make with that statement then.
