I have the same problem he describes, I have the change volume with buttons
enabled and the volume added to the rotor. If I turn up VO volume during a
speakerphone call to 100% I have to be in a very quiet place and hold my
phone close so I can even hear Voiceover. I have perfectly good hearing by
the way and this is definitely very inconvenient. I make a lot of calls with
headset, but for those times I don't and need to press numbers during a call
or listen to a menu it's a pain in the neck.

I think this seems to be one of those bugs which appear only on certain
phones. It's like when some people always have issues with the iPhone
dropping off WiFi or the Voiceover going silent thing in iOS 5, I have never
had these issues, but I'm sure they exist. Same with some people reporting
the battery life on their 4S is worse in iOS 6 than before, it's the
opposite for me and others.

I'm not sure   this is a bug.  I'm not seeing this behavior at all on my
iPhone 5 or 4S running IOS 6.  Actually, In IOS 6, for me at least, they
have increased the volume of voiceover when on a call and the phone is
placed on speaker.  Its so loud, I had to turn down Voiceover.
