Hi, I'm not sure this is a bug. I'm not seeing this behavior at all on my iPhone 5 or 4S running IOS 6. Actually, In IOS 6, for me at least, they have increased the volume of voiceover when on a call and the phone is placed on speaker. Its so loud, I had to turn down Voiceover.
