Hi Ricardo,
this is getting weirder and weirder, as my problem actually started after 
updating to iOS six.
Someone help me rationalise this.

  Yeah. I hear ya.  Its just odd that I use to have that problem but, after 
updating to IOS 6, its fixed.  Meaning, Voiceover is now loude enough to be 
heard over the call on speaker phone.  I Don't know, I guess its just one of 
those things.
