Any GOOD host will have such a form.

Brainstorming about what all the possibilities might be. Just brainstorm ideas 
about what could be done.  Do this with the client.  Then begin to decide 
exactly what it is that the customer wants; ask the customer. Then create the 
flow chart and get the customer to sign off on it. Then give that to a programmer.

Or you can do it like everyone else. Just sort of tell a programmer that you 
want an email form and give him an email address. He then just does that and 
then you tell him to do this and that and then the customer wants to do 
something else and then .....ugh.
