I've used cgiemail in the past. It allows for very easy formatting of the
resulting email...better than most other form-to-email scripts. I have had
one problem setting it up in the past, which ended up being that I didn't
have a /var directory setup as a temp dir. That was an install I did over a
year ago, though, so maybe things have changed.
