Gee, this is just what is beginning to irritate me to no end.  In my 
personal life I will do a lot of wild and crazy things but at work I 
like things to be organized.

I have been looking for templates that must be used and filled in for 
any IDEA.  I hate getting a request for work where I have no idea 
where I am in the process of the project.  It would be nice to see a 
project definition, job assignment, job flow, --- and expected 
result.  I might even go so far that a job should be designed at 
least on paper (if not html) with all the pieces laid out for all to 
see.  After that is done, the originator of the job then assigns the 
work to make the design come to life.  As it is now it seems just 
hard to determine who is in charge of a project.

ps, what sort of books would give me practical advice on developing 
projects --- not theory but more like a workbook with templates --- 
or whatever you might call them.
