Is the problem account a Comcast account? I have four different Comcast accounts and one of them for a few days now has been downloading old messages from the server. Unfortunately, when a POP server is reset then the message IDs that are unique to the messages on the server can also be reset. To Entourage, these messages appear to be new and therefore it downloads them again. You cannot control this and it's not Entourage's fault. The best you can do is reduce the number of days you keep messages on the server under Tools --> Accounts... --> <your Comcast account> --> Options. To clean up when this happens, I select View --> Unread Only, select all the messages and delete. This, of course, only works if you have all your current messages marked as read to begin with.
