You should look for multiple copies of the tuner and tuner-ivtv modules.
IIRC correctly, the one you want is just tuner.ko, and should have a
recent modification date.  Might also want to remove the alias tuner
tuner-ivtv line as well.

Similar recommendations for the other ivtv specific modules (e.g.
tveeprom, msp3400, etc.)
