Basically, just edit ivtv-fileops.c and remove the two tda9887 == 0 blocks and see if it fixes your missing audio. I'm thinking that me adding that code was a bad idea (I thought it wouldn't break anyone).

I've had the same problem with my PAL version of pvr-500mce (tuner=56), so here's my five cents about that patch:

For me the patch didn't fix the missing sound on tuner 0, it fixed another problem, where my sound would disappear from the first time I changed channel, until next time the modules were reloaded. Change channel = loose sound. That was the same on both tuners.

at one point I also had the missing sound on tuner 0, but it wasn't fixed by this patch - I don't know what fixed it, but I've used that patch on all the versions I tested it - going back and forth between the versions... suddenly it just worked... got no clue why.

Allthough I think you should try the patch anyway, it might work for you - at least it will fix the other problem I mentioned :)
