The desktop is just as vulnerable to anything saved to it compared with saving the same file to another location.
I don't have any virus programs on my computer and it may be the case that your AV program has set up some sort of interim quaranteen on the desktop, but I've never heard of such a thing. Does the maker of you AV program suggest you save your attachments to the desktop instead of someplace else? I would think that ANY new file save ANYWHERE on your computer regardless of where it came from (floppy, email, CD) would be subject to the same scan and 'guarantee' that it's clean.
