Hi Jennifer,

We have an employee code of conduct policy; however, we do not require written 
acknowledgement.  This may change as the policy is currently under 
review/revision.  We do have several compliance training videos that we push 
out on other topics that provide records of employee acknowledgment.

I am doing a review of our institution's Code of Conduct and was wondering if 
your institutions require employees to attest to or acknowledge the 
institution's Code of Conduct. If yes, how often is it required (one time or 
recurring) and how is it obtained (manual or electronic database platform)?
