The University of Washington does not require attestations regarding its 
Student Conduct 
Code<http://www.washington.edu/admin/rules/policies/WAC/478-120TOC.html> or its 
General Conduct 
Code<http://www.washington.edu/admin/rules/policies/WAC/478-124TOC.html>; 
however, both are Washington Administrative Code rules that apply similar to 
any other state agency rules or statutes.

your institutions require employees to attest to or acknowledge the 
institution's Code of Conduct. If yes, how often is it required (one time or 
recurring) and how is it obtained (manual or electronic database platform)?
