Cornell does not require employees to complete an attestation on this 
particular policy, although our Campus code is widely distributed and 
publicized.

your institutions require employees to attest to or acknowledge the 
institution's Code of Conduct. If yes, how often is it required (one time or 
recurring) and how is it obtained (manual or electronic database platform)?
