Can you try to reduce in the asound configuration?

Actually, I've raised in asound.conf from 103 to 118 in the
Earpiece_incall section, 'cause plain AT+CLVL=255 seemed to be not
enough...
But I think that you are near the saturation of volume.

Now I've remembered there's TonePicker, so I'm installing it and will
try to find the best combination.
