Did you check the permissions like Sandra suggested? If it shows the
entry point being checked then I'd verify that the permissions are there
and it's not set to hidden. 
Fixed/Read/Float licensing doesn't really come into play on this, it's
the permissions of the form and the entry point being checked.

open the Remedy Support form and re-enable the Entry Point to that form
in the View Properties.  That will make it visible on the Home Page.
Then inform that person that they owe you (lunch, drinks, chocolate,
