Katherine, I haven't read most of this thread, so if I repeat some of the things that were already suggested, ignore them.
Have you checked to see if they have disabled the entry point for that form? If they have done that, re-enable them.. Check for permissions on the form. If the groups that are to see the forms were removed from the permissions, the entry point would not be seen in the application list field. Also check to see if they have changed the permissions to that form from view to hidden. If I remember right even though you have entry points enabled if you change the permissions of the group or groups from view to hidden, the links disappear from the Application List field. Check to see if the there was a mass change of permissions to the users. Make sure they have the appropriate roles and permissions so they can view that form. I kinda have this problem. One of my admins decided to change something in Remedy that they didnt like. I say "something" cause I am not sure what was changed. I only know the end result. The problem I am having is when you open up Remedy and the HomePage is displayed. We have the usual "Remedy approver", "Remedy Management" stuff like that. So this morning I discovered that none of my client users had "Remedy Support" as a choice anymore. I am baffled on how to get that back. I know there is an easy answer to this but I am dumbfounded.
