Although this is not an option in Ohio, some states have even adopted the
MPAF as their designated quarterly assessment form. (NC and NJ I believe)
Note this line from the RAI manual: "If a facility continues to submit a
full MDS assessment for Medicare, the extra MDS items (those not on the
MPAF) will be ignored and will not be edited or stored in the State MDS
database." Page 2-32
Even if you complete the full assessment the state is only accepting or
reading the MPAF items when you submit. If you're having implementation
issues in your particular software I'd suggest contacting their help desk
for tips / ideas, or reviewing the user's manual, etc.

I'm the MDS Coor for a 27 bed hosp based SNF in Ohio with a 7-10 day LOS. We
use the MPAF form for all 5 day PPS assessments. We have been doing this
since the MPAF was ready for use. We have a consultant here stating that
MPAF is difficult to use, causes lots of problems. I would like to know what
others are doing. There are 3 of us here that have been doing the MDS for
years. And with averaging over 100 assessments a month the MPAF has been a
