Nick, You have been told what you need to do and yet you don't do it. You continue to do what doesn't work and are surprised when it doesn't work a 2nd, third, even forth time. It's a hardware issue. You have the wrong cable. Make the right cable and it will work. Getting the right adaptor is like getting the right cable. Getting the wrong adaptor is like getting the wrong cable.
The problem is you are not setting the sense pins properly, which is to say you are not telling your computer what monitor is hooked up. So it doesn't know what to do. So either buy an adaptor with dip switches (like the sony one), or get out your soldering iron and make a cable (it is actually pretty easy to do). You could, if you wanted, just modify the cable you have.
