First question is, if it works on 8.6, why do it on 9.1? Just do it on 8.6 then transfer the rebuilt database back to 9.1 (or am I missing something here). Next, if it gets close, but doesn't quite finish, do two things. 1: run a disk first aid utility to verify the drive is good.
