The repair permissions process uses the information in /Library/Receipts to
restore permissions to what those receipts say they should be set to. Only
installers that make user of Apple's Installer application (.pkg or .mpkg)
place receipts in /Library/Receipts and are, therefore, the only once that
have a say in what is or isn't "repaired".

Saying that repairing permissions "only affects those applications..." is a
little misleading, though, as the installer may modify permissions on folder
that doesn't natively belong to it. Say, for example, that an installer (or
whatever) modifies the permissions on the /tmp folder. That would cause all
sorts of problems. Running a permissions repair should sort that out.
