You know, I haven't been following this conversation very carefully, so I don't know the viability of the suggestion I'm going to throw out (nor if it has been discussed, yet) - and I have only a minute in between the jury duty I served this morning and going back to work, so it won't be very detailed:
Can the runway lighting location be given a z-offset that is a percentage of the aircraft height above ground level, up to a certain maximum?
