You could pack something perfectly, and UPS will disallow the claim due to "insufficient packaging" because it didn't comply with every bit of UPS's definition of "sufficient packaging" (say, 1.9" of packing material between the item & side of box instead of 2"). I once had a claim disallowed due to "insufficient packaging," when it was quite obvious (to me) that even "sufficient packaging" adhering perfectly to UPS' packing rules would not have protected the item from the forklift blade that was run through the side of the box (evidenced by the forklift-blade-sized hole in the box). Unfortunately, we had no idea who made the hole (the shipping end, the recieving end, or UPS), though my vote was for UPS, as boxes shipped by them appear to arrive with some form of damage on a more regular basis than those sent via other shippers.
