audio quake is a great example of what can be done, however one thing that struck me about the game when I played it is that access would only work in specifically designed maps.

I did deathmatch with some bots and tried your starwars mod, however when I tried to play the main quake game I found it wasn't possible with the access tools. This might have been me I don't know.

So, while I perfectly agree with you that holding a black and white scale of accessibility is a bad idea, at the same time there do seem to be limitations even when very successful access tools are created for a game.
