I have also encountered this and have had other players report invalid
custom models. To recreate is just as ics has explained.

custom models, like "Swing's cool skins" that may or may not have

the errors. You will not see them if you play on custom model server
without pure or a single server but if you join any pure 1/2 server

This is a very old bug and has not yet been fixed. There
have been many topics posted here but Valve has yet to
acknowledge this issue.
