I think in that situation we need to have the server maxplayers reading
as "14", and there needs to be a standard cvar (server rule) returned
that says how many additional private slots there are available ("2"),
emulating the behaviour of Q3's sv_privateClients system.

As far as I can tell that cvar does nothing. Anybody know what it's
meant to do?
It's not exported as a server rule to queries, and changing it does not
change the maxplayers reported in queries.
