Regarding wrong IODF... DS QL,LIST and/or DS QL,LISTALL may be useful. Do you see 00001?

I guess I just assume "hardware issue" first since us sysprogs always know what we are doing and never make mistakes. :-) Check with Dell/EMC. Also, for all the implementations I have been involved with, Dell/EMC typically provides an "Architecture level 2" and "Architecture level 3" document and they have had the library ids and port IDs identified for all the libraries being implemented within the DLm(s). If you have a document like that check it and see if they documented something else.
