I just saw this item after posting my  other  comments about  fail safe

A very interesting piece in today's Seattle Times:

Apparently, when the ship lost electrical power, its prop went into 
reverse, causing it to take water over the stern and vastly 
complicating the abandon ship procedures.

Some but not all propellor positioning  systems start from one end of a rack 
position as full reverse, then half way is neutral and  the full extension is 
forward. 
The accident description in the report sounds as if the rack positioning gear 
went to full OFF when the power failed. 
It would have been better if that propeller positioning device had "failed in 
mode" and left the prop blades in whatever position they were in at the time of 
power failure. 
Curiously enough Lloyds  Standards require dual  redundant and continuously 
powered busses feeding such things as steering and propellor positioning gear.  
ABS does not stipulate redundancy  in exactly the same way.  Hmmmm! Time for a 
standard rewrite?
